Documents Required:
- SIM (in case of lost SIM, GD copy is required.)
- Original Ownership / Subscription Paper with 1 recent passport-size photograph
- If the original owner is unable to collect the SIM, an Authorization letter can be given to another person on behalf of owner.
- Signature on the letter should match the signature on the original subscription.
- If new SIM needs to be issued, an additional charge of Tk. 68 is required.
